Goals of Care Discussions

Children with medical complexity (CMC) often have multiple life-limiting conditions with no unifying diagnosis and an unclear prognosis and are at high risk for morbidity and mortality. Advance care planning (ACP) conversations need to be uniquely tailored to this population.

Supportive Teams for Advanced Care/Difficult Life Stages
Palliative Care

The program provides consultative and clinical services to children with life-limiting illnesses and progressive decline in health status. The palliative care team works closely with other CHEO services and community partners, including Roger Neilson Children's Hospice, the pediatric hospice, located on the grounds of CHEO.

Quality of Life and Advanced Care Program

Quality of Life & Advanced Care (QoLA Care) at McMaster Children’s Hospital serves families affected by serious pediatric health conditions which impact quality of life. Patients may be a fetus, infant, child, youth, or young adult. Our goal is to optimize quality of life for our patients and their families, now and in the future.

Paediatric Advanced Care Team

The Paediatric Advanced Care Team (PACT) meets with children and their families when a child is facing a serious illness to provide an extra layer of support.
